Autofill Ontario Power Of Attorney Kit
Prepare your power of attorney without retyping your personal details
Ontario power of attorney documents — for property and for personal care — both ask for the same grantor and attorney name and address fields. This guide is an example of storing those repeating fields in hivi.
Official form and source
Start from the official Ontario source and always use the current version — requirements can change:
- Service page: Make a power of attorney — ontario.ca
What these documents are about
A Continuing Power of Attorney for Property lets your chosen attorney manage financial decisions if you become incapable. A Power of Attorney for Personal Care lets your attorney make health and personal decisions. Both require witnesses and must meet the formal requirements of the Substitute Decisions Act.
Before you start: what to gather
- Your full name and address (as grantor).
- Your attorney's full name and address (one or more people).
- Two witnesses who meet the eligibility criteria.
Confirm the current form requirements and execution rules on ontario.ca.
How hivi makes it faster
- Store your details and your attorney's details in hivi. Add names and addresses for both.
- Autofill the grantor and attorney fields. Use the hivi Chrome extension on the forms, or fill PDFs from your saved profile inside hivi.
- Arrange eligible witnesses yourself.
- Execute the documents with witnesses present.
Why do it in hivi
Grantor and attorney details repeat across both POA documents and any amendments. Stored once in hivi — in Canada, with no third-party AI — each document starts mostly filled.
