Request an ODSP internal review without retyping your details

If you disagree with an Ontario Disability Support Program decision, you can ask for an internal review — and the request form repeats your name, date of birth, address, and member ID. This guide is an example of storing them once in hivi.

What this form is about

An internal review asks the ministry to reconsider an ODSP decision — such as eligibility, benefit amount, or a specific denial. You must usually submit the request within 30 days of the decision letter. Mail or fax it to the address shown on that letter.

Before you start: what to gather

  • Your name, date of birth, and current address.
  • Your 9-digit ODSP member ID from your decision letter.
  • The decision letter and a clear explanation of what you disagree with.
