Autofill Ontario Blind Low Vision Program

Register for the Blind-Low Vision program without re-entering your family details

Registering a child for Ontario's Blind-Low Vision Early Intervention Program repeats the same parent name, address, and contact details you enter on other child services and health forms. This guide is an example of storing those repeating fields in hivi.

Official form and source

Start from the official Ontario source and contact a BLV program location for registration:

What this program is about

The Blind-Low Vision (BLV) Early Intervention Program provides family-centred services and supports for children from birth to school entry who have a visual impairment. Specially trained consultants work with families in the home. Registration is through BLV program locations across Ontario.

Before you start: what to gather

  • Child's full name, date of birth, and visual impairment diagnosis.
  • Each parent's or caregiver's name, address, and contact details.

Confirm current program locations and eligibility on ontario.ca.

How hivi makes it faster

  1. Store your family details once. Save your child's and your own name, date of birth, and address in hivi.
  2. Autofill identity fields on intake forms. Use the hivi Chrome extension on online forms, or fill PDFs from your saved profile inside hivi.
  3. Provide diagnostic information yourself — medical details cannot be autofilled.
  4. Complete registration with the BLV program location.

Why do it in hivi

Parent and child identity details appear across BLV registration, school registration, and benefit applications. Stored once in hivi — in Canada, with self-hosted AI — each form starts mostly filled.
