When your device needs an upgrade, modification, or new option, the Assistive Devices Program requires a supplementary application form beyond the original funding request. What this form is about

This application requests ADP funding for a change, modification, or new option added to a device you already have funding for. It documents your identity, links to the original approved device, and captures the modification details from your authorizer and vendor.

Before you start: what to gather (your section)

  • Your full legal name and current Ontario address.
  • Your Ontario health number.
  • Original ADP device information — confirm what details are required on the current form.

Arrange for your registered ADP authorizer to assess the modification and for your vendor to specify the change.
