What this form is about

Form 18 is the application to the Consent and Capacity Board to review a finding that someone is incapable of managing property (Substitute Decisions Act, subsection 20.2(1)). It is not a guardianship application and not a power-of-attorney form — it challenges an existing incapacity finding.
