The renewal keeps your Ontario health coverage current. It confirms who you are, that you still live in Ontario, and links to your existing health number.

Before you start: what to gather

  • Your full legal name and date of birth.
  • Your current Ontario address.
  • Your existing health number.
  • Documents proving your identity and Ontario residency (confirm the current list on ontario.ca).
