What this form is about

If a birth was not registered within the required period, ServiceOntario issues a delayed registration package. The completed package is mailed to the Office of the Registrar General in Thunder Bay.

Before you start: what to gather

  • Each parent's full name, date of birth, and current address.
  • The child's full name, date and place of birth.
  • Supporting identity documents — confirm which are required on ontario.ca.
